Sofosbuvir is a nucleotide inhibitor of the HCV NS5B polymerase and has demonstrated potent in vitro activity against all six genotypes of HCV. Sofosbuvir was approved by the FDA on December 6, 2013, for use in combination with other agents for the treatment of chronic hepatitis C infection in adults. The recommended dosage is 400 mg per day taken orally with or without food.

The clinical efficacy and safety of sofosbuvir was evaluated in two Phase III clinical trials: one trial evaluated sofosbuvir plus ribavirin (RBV) versus placebo plus RBV; and one trial evaluated sofosbuvir plus peginterferon alfa-2a (PegIFNα-2a) versus PegIFNα-2a alone. In both trials, sofosbuvier demonstrated statistically significant improvements in rates of sustained virologic response 12 weeks after completing therapy (SVR12), which are considered to be cure rates. In patients treated with sofosbovir/RBV, SVR12 rates were 89%–90%, compared to 31%–40% among patients who received placebo/RBV therapy. In patients treated with sofobuvir/PegIFNα-2a, SVR12 rates were 95%, compared to 78% among those who received PegIFNα- 2a alone
