Ampicillin is one of the most commonly prescribed antibiotics in the United States and has been around since 1961 when it was first introduced into clinical practice. Ampicillin belongs to the class of drugs called beta-lactam antibiotics which also includes penicillins and cephalosporins. Beta-lactams work by inhibiting bacterial cell wall synthesis which ultimately leads to cell death. Ampicillin has activity against both Gram-positive and Gram-negative organisms making it useful for treating a variety of infections including respiratory tract infections, urinary tract infection, skin and soft tissue infection, intraabdominal infection, sepsis, and meningitis. It is important to note that although ampiclllin exhibits broad spectrum antibacterial activity it does not cover all potential pathogens so culture and susceptibility testing should always be performed when initiating therapy with this agent. Additionally. due its low cost. oral form. lack of toxicity.and long half life; ampiclllin is often preferred over other agents for outpatient treatment regimens. In general.ampcillin can be dosed every 6 hours orally OR intravenously OR rectally depending on indication. The recommended dose for adults with normal renal function is 500 mg every 6 hours orally OR 1 gm every 12 hours intravenously. Although there are few data on use during pregnancy.ampcillin appears safe at standard doses ; however caution should be taken when using higher doses especially late in gestation.
